How do Care2U & Care2U-Rx work as one integrated team?

Our providers and pharmacy work together seamlessly in real time, so there's no clunkiness in your process to receive medicine and care. When a medication adjustment is needed, our pharmacy team knows about it immediately and acts. When something changes, everyone on our team is informed. That kind of unified communication means fewer errors, faster responses, and a smoother experience for both your patients and your staff.

No more waiting on hold or hoping your medication arrives on time. At Care2U, that's simply how we operate.

How do you communicate and coordinate with facility teams?

We believe in staying ahead of issues instead of just reacting to them. If anything changes or a concern arises, we reach out to your designated contact right away. You can always call us directly and speak with a real team member, whether that's our pharmacy or medical staff. We also offer a secure messaging app so communication is simple and fast. Think responses within hours versus days or weeks.

How does medication packaging work?

We package monthly medications specifically for your facility to make med passes smoother, faster, and more accurate. It's one less thing for your team to manage, and one more way we help reduce the risk of errors.

How often does the nurse practitioner come to the facility/home?

Typically, routine visits occur once a month. Additionally, acute visits are available as needed because timely intervention is key to successfully improving care outcomes.

Do you only work with facilities, or individuals as well?

Both. We proudly serve assisted living facilities, group homes, and other care settings, as well as patients in their own homes. Wherever care is needed, we show up.

How does Care2U improve health?

When primary care, pharmacy, and behavioral health work together as one unified team, patients don't fall through the cracks. Our integrated approach means health changes are caught early, medications are managed with precision, and consistent care keeps small issues from becoming bigger ones.

● Early identification of health changes before they escalate

● Consistent, routine care that builds a real picture of each patient's health

● Precise medication management through our combined provider and pharmacy team

● Fewer hospitalizations and emergency visits

● Genuine improvement in overall quality of life

Do you deliver medications, or do we pick them up?

We deliver directly to your facility or to a patient's home. No pickups or extra coordination required. It's all part of making care as seamless as possible for everyone involved.

Is there any additional cost?

In most cases, no. Our services are covered through insurance, so families and facilities can access the unified care they need without worrying about added costs. Our team is happy to walk you through what's covered during your first conversation with us.
