Rural Kentucky facilities were managing pharmacy and medical care through disconnected vendors, with no one accountable for the whole picture. Care2U changes that.
Care2U combines long-term care pharmacy and mobile medical services under one team, one mission, and one standard of care. The pharmacist and nurse practitioner are communicating daily, so they catch problems before they become crises. For the facilities we serve, that means fewer delays, fewer errors, and fewer calls chasing answers that should already be coordinated.

We built Care2U because we kept seeing the same problems: good facilities with caring staff dealing with a system that made coordination harder than it needed to be.
This is what it looks like when pharmacy, primary care and behavioral health finally work as one team. We think you'll recognize the difference right away.
Care2U started as a pharmacy 17 years ago. The longer we worked inside these communities, the more we saw the same problems. Delayed medications.
Providers not showing up. No one accountable for the full picture. So we built something better.

Long-term care pharmacy built from the ground up in Kentucky, focused on getting medications right and showing up for the communities others overlooked.
Providers not showing up. Prescriptions delayed because pharmacy and prescriber weren't talking. Facilities managing two vendors with no one accountable for the whole picture.
Mobile primary care, then behavioral health. One team, one mission. Because the people we serve deserved better coordination than the system was giving them.
Care2U partners with assisted living facilities, memory care communities, group homes, and SCL agencies supporting individuals with intellectual and developmental disabilities. These populations have complex, ongoing care needs. They deserve partners who understand that, not vendors who treat them like a standard account.
Everything we do comes back to four commitments.
Delivering Unified Pharmacy and Medical Care Guided by the Compassion of Jesus.
One team. Unified care.
Healthier communities.
We treat every patient with dignity, every partner with respect, and every community as if it were our own.
We do what we say. When something goes wrong, we own it and fix it fast.
Medications arrive when expected. Providers show up when scheduled. Communication doesn’t go dark.
We hold our work to a high standard, not because we’re perfect, but because the people we serve deserve our best effort every time.

The result is fewer delays, fewer errors, and less time your staff spends managing vendors instead of caring for residents.
Packaged and ready, without your staff chasing refills.
Nurse practitioners come to residents who can't leave the building.
You'll hear from us the same day, so you're not waiting or wondering.
Let us show you what coordinated care actually looks like. If it works, you’ll see it. If it doesn’t, no pressure.
Most of the people we work with today started exactly that way.
Response within one business day. No commitment required.
